Our Legal Duty

Under the Health Insurance Portability and Accountability Act (HIPAA) and Michigan health privacy laws, Neu Healthcare (including Neu Urgent Care, Neu Family Care, Neu Hope) is legally required to maintain the privacy of your Protected Health Information (PHI), provide you with this notice of our legal duties and privacy practices, and notify affected individuals following a breach of unsecured PHI.

How We May Use & Disclose Your Health Information

  • Treatment: To provide, coordinate, or manage your healthcare services among our medical staff, specialists, laboratories, or external care teams.
  • Payment: To bill and collect payment from insurance companies, Medicaid, Medicare, or self-pay accounts for services rendered.
  • Healthcare Operations: To support quality improvement, provider evaluations, audits, compliance reviews, and administrative duties.
  • Required by Law: For public health monitoring, health oversight audits, court orders, law enforcement requests, or reporting as mandated by State of Michigan laws.

Substance Use Records (Neu Hope)

Records generated by Neu Hope are protected under federal law (42 CFR Part 2). Disclosure of substance use disorder records requires specific written patient authorization, except in defined emergency situations or explicit court orders.

Your Health Information Rights

  • Inspect and Copy: You have the right to inspect and receive an electronic or paper copy of your medical records.
  • Amend Records: You may request corrections to medical records you believe are inaccurate or incomplete.
  • Accounting of Disclosures: You may request a record of non-routine disclosures made of your PHI.
  • Request Restrictions: You may ask us not to use or share certain PHI for treatment, payment, or operations.
  • Confidential Communications: You may request communications via specific phone numbers, emails, or mailing addresses.

Privacy Complaints & Contact

If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer or with the Secretary of the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
